People who aren't on antiretrovirals early and consistently have shorter lives and more complications, which means 3 in 10 people with HIV still die from AIDS-related complications.

Nearly 30% of individuals with HIV still die from an AIDS-related cause, often from lack of or lateness of treatment or lack of medication adherence

AIDS complications is still the most common cause of death among people with HIV 

Non-HIV related cancers are the most frequent causes of non-AIDS deaths (they kill 15% of people with  HIV)

Liver disease is second most common cause of death, mainly due to hepatitis (it kills 13% of people with HIV)

And then cardiovascular disease is the third leading cause of death at 11%
